If your need is urgent, please call (435) 656-5323 to request a same-day appointment. Requests are handled during regular business hours. Our business hours are:

Monday – Thursday  8:30am – 5:00pm

Friday  8:30am – 2:00pm

Cancelling/Rescheduling an Appointment:

From time to time schedules can change and we understand. Please call our office 24 hours in advance to let us know if you need to cancel or reschedule your appointment.

We may also choose to discharge a patient for habitual late cancellations and or rescheduling. We want to serve all of our patients in a timely and efficient manner and effective communication is the best way to accomplish this.

Contacting a Physician after hours:

We have a 24-hour, 7-day-per-week answering service. Call (435) 634-6295. All emergent calls received after hours will be responded to individual need of the patient.

Telehealth Appointments:

We offer telehealth appointments online so you don’t have to make the trip to our clinic. Please contact us to schedule your appointment and then the login page for your appointment is here.

Renewing a Prescription:

Please call your pharmacy to request a prescription refill and the pharmacy will fax or contact the office. Prescription renewals are handled during regular office hours only. Please allow at least 48 hours during the business week to accommodate your request, as each request has to be approved by a physician and all physicians are not in the office every day.

Lab/Radiology Results:

Our staff will call with all results for labs and radiology, however if you have not heard from us within 4-5 days of your visit please contact the office.

Pain Management

Our providers do not prescribed pain medication on a long term basis.  If your case is one that requires long term use of pain medications we will refer you to a pain clinic and work with them to help you get the treatment you need.

Your Account:

We accept cash, personal checks, Visa, American Express, Discover and MasterCard in our office. We offer a discounted rate for cash pay patients.

If you have questions about your bill, please call your insurance about any Deductibles or Co-payment amounts. If you need to talk to the billing department please call (435) 656-5323 .
